Position: APP Manager NP or PA Medical Hem Oncology Office Practice Staten Island 8 hr Days

Job Overview

As a HOME HEALTH LPN, you will provide professional nursing services under the direction of an RN in compliance with the state's Nurse Practice Act, state licensure/certification requirements, applicable healthcare standards, and company policies. You will offer skilled nursing interventions for patients or clients, including treatment of illness, rehabilitation, preventive care, and teaching activities as part of individualized care plans.

Responsibilities
  • Provide professional nursing services in compliance with state regulations and company policies.
  • Deliver skilled nursing interventions for patients’ treatment, rehabilitation, and preventative care.
  • Apply clinical knowledge to facilitate problem resolution and achieve individualized patient goals.
  • Consult with RN supervisors regarding changes in plans of care and accept verbal orders from physicians, as allowed by state law.
  • Implement individualized goal/outcome driven nursing plans.
  • Coordinate care by maintaining timely communication with patients, physicians, clinical managers and other team members.
  • Maintain professional conduct regarding confidential information and share data only when appropriate.
  • Communicate effectively to ensure supply and equipment availability.
  • Prepare, submit and maintain required documentation, including shift notes.
Qualifications
  • Graduate of an accredited Licensed Practical Nursing program or accredited School of Vocational Nursing.
  • Current nursing license in the practicing state.
  • Valid driver’s license, auto insurance, and reliable transportation.
  • Current CPR certification.
  • Minimum two years experience as an LPN/LVN, preferably in a home health or hospice setting.
