Key Responsibilities

• Deliver direct patient care based on individualized care plans.
• Assess patient conditions and implement appropriate nursing interventions.
• Administer medications and treatments as prescribed.
• Monitor and document patient progress, vital signs, and responses to treatments.
• Collaborate with physicians, therapists, and other healthcare professionals to optimize patient care.
• Educate patients and families on healthcare plans, medications, and post-discharge care.
• Maintain accurate medical records and adhere to documentation protocols.
• Ensure compliance with all healthcare regulations, infection control standards, and safety policies.
• Provide compassionate end‑of‑life care when necessary (for home health or hospice cases).
• In-home care may include wound care, IV therapy, ventilator management, and patient education.

• Active Registered Nurse (RN) license in [Insert State].
• Minimum of 1 year of experience in a healthcare facility or home health setting (preferred).
• Current BLS/CPR certification (ACLS/PALS a plus).
