Role Overview

The Wellness Nurse RN is responsible for overseeing resident health, conducting assessments, monitoring health trends, and ensuring medication safety and regulatory compliance within a senior living community.

The Wellness Nurse RN is responsible for overseeing resident health, conducting assessments, monitoring health trends, and ensuring medication safety and regulatory compliance within a senior living community.

• Perform assessments at move-in, 3-day, semi-annual, and change-of-condition periods
• Conduct monthly wellness visits and follow up on falls, incidents, and hospital returns
• Transcribe physician orders and maintain accurate MAR/TAR documentation
• Complete medication audits and oversee medication program compliance
• Partner with families, physicians, and the Resident Care Director
• Promote infection control and survey readiness
• Provide clinical guidance and training to care staff
• Serve as a proactive clinical resource to prevent decline through assessment, early intervention, and documentation oversight

• Active RN license in good standing
• Graduate of an approved nursing program
• At least 1 year of experience in senior care or a clinical setting
• Strong assessment, regulatory, and communication skills
