What You Bring (Requirements)

  • A minimum of one year’s experience in working with the frail or elderly population required.
  • Graduate of an accredited school of professional nursing
  • Current state Registered Nurses (R.N.) License, Cardio Pulmonary Resuscitation (CPR) certification required
  • Must have a valid driver’s license, proof of insurance and have means of transportation.

What You’ll Be Responsible For:

Conducts an in-person comprehensive initial nursing assessments in the PACE clinic and in participant’s homes with new PACE enrollees. Coordinates with the Interdisciplinary Team to develop a comprehensive plan of care for each participant.

  • Conducts an in-person nursing re-assessments in the PACE clinic and in participant’s homes semi annually and as needed.
  • Participates in the nursing on-call rotation.
  • Functions as a member of the Interdisciplinary Team. Maintains regular attendance at, and participates in Interdisciplinary Team meetings; communicates participant changes, collaborates on plan of care decisions and coordination for twenty-four (24) hour care delivery.

As a Program of All-Inclusive Care for the Elderly (PACE) provider, our interdisciplinary teams deliver personalized, comprehensive care tailored to each participant’s needs. Located in Lanham, this community offers convenient access to Washington, D.C. while maintaining a welcoming, suburban feel.

Qualifications
