Requirements:
- Current State License as a Physical Therapist
- Proof of current CPR certification
- Current driver's license. Must have reliable transportation and insurance
- 1-2 years experience as a Physical Therapist in an acute care or rehabilitation setting
- Previous home care experience - Preferred
- Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program
Roles & Responsibilities:
- Perform clinical assessments to identify patient needs and appropriateness of care.
- Document and complete a Plan of Care (POC), progress notes, and other clinical records continuously.
- Plan and provide treatment for neuromuscular, neurological, and orthopedic disorders based on evaluation.
- Educate patients and families regarding therapeutic procedures tailored to their needs.
- Collaborate with an interdisciplinary team to ensure comprehensive patient care.
- Apply infection control concepts to prevent contamination during patient care activities.
