Center Well Senior Primary Care -  - Responsibilities:
Provide direct patient care under the direction of the RN and according to the Plan of Care;
Assist with self-administered medications and monitor patient medication intake;
Measure and report vital signs (temperature, pulse, blood pressure);
Maintain patient personal hygiene and assist with bathing, grooming, oral care, nail/foot care;
Assist in feeding and monitor nutrition and fluid intake;
Position patient for meals and assist with feeding as needed;
Assist with toileting and ostomy care as required;
Assist with mobility, transfers, and ambulation per Plan of Care;
Document care provided and communicate changes to the supervisor;
Maintain infection control practices and provide a clean, safe environment
