Position SummaryThe Samaritan Center – Simi Valley seeks a Senior Lead Case Manager to provide strategic leadership, supervision, and oversight of all case management services. This is a senior-level, hands‑on leadership role responsible for guiding, mentoring, and supervising Case Workers while maintaining a small caseload of complex clients.
The Senior Lead Case Manager ensures high‑quality, trauma‑informed, and coordinated services for individuals and families experiencing housing instability, homelessness, food insecurity, and related challenges. This role requires strong staff management experience, sound judgment, and the ability to lead programs, people, and systems with professionalism and compassion.
Key Responsibilities Program & Case Management Leadership- Provide overall leadership and oversight of case management services, ensuring consistency, quality, and compliance with agency, county, and funder requirements.
- Serve as the primary supervisor for Case Workers, including coaching, mentoring, performance feedback, and support in complex cases.
- Develop, update, and implement case management policies, procedures, intake processes, and service standards.
- Ensure best practices in trauma‑informed care, client‑centered planning, and harm‑reduction approaches.
- Represent the agency at Pathway to Homes (Ventura County CoC) and other required county or collaborative meetings, advocating for clients and programs.
- Act as a senior liaison with community partners, housing providers, county agencies, and service organizations.
- Supervise, support, and oversee the work of 2–4 Case Workers, including case review, workload management, and documentation quality.
- Review daily logs, time tracking, and HMIS entries for accuracy and compliance.
- Provide guidance and leadership during crisis situations, street outreach, or high‑risk client interactions.
- Foster a collaborative, accountable, and mission‑driven team culture.
- Identify training needs and support professional development of case management staff.
- Maintain a limited caseload focused on high‑acuity or complex clients.
- Conduct comprehensive intakes, assessments, and individualized action plans.
- Coach and support clients in accessing housing, benefits, identification, employment resources, food assistance, healthcare, and recovery services.
- Advocate for clients within systems and with external agencies.
- Monitor client progress and adjust service plans to support long‑term stability and self‑sufficiency.
- Ensure accurate, timely, and complete documentation in HMIS and client files.
- Oversee HMIS reporting, data quality, and compliance with Ventura County standards.
- Utilize HMIS and internal data to evaluate program effectiveness and outcomes.
- Support the Executive Director and Board Members with grant applications, reporting and audits.
- Prepare documentation and reports required for grant compliance and funding sources.
- Oversee internal reporting of client supportive services – Stat Sheet.
