Schedule:Per Diem (12 RVE per week) must work 3 days per week:
Mon, Wed, Thur or Tues, Thurs, Sat. Non-traditional schedules are available, e.g., Thursday-Monday.
Area #45 covers the following cities:
Agoura Hills, Calabasas, Moorpark, Newbury Park, Oak Park, Simi Valley, Thousand Oaks and Westlake Village.
Must be able to train for 5 days Mon‑Fri 9am‑5:30pm in San Ramon (onsite).
Position Summary:The Home Health RN monitors a patient’s overall health and, in partnership with a primary care physician, develops an individualized plan of care. Each RN functions as a case manager and works independently to deliver quality healthcare, including customized care, patient education about disease management, and specialized services such as simple to complex wound therapy, post‑surgical care management, and ostomy care management.
We provide an entire internal operational support team for our clinicians to handle the tasks of pre-populating medication reconciliation, tracking DME orders, obtaining MD signatures, coding, lab follow‑up, etc., truly allowing clinicians to function independently and plan patient visits around other commitments if needed. The home care setting allows quality time with patients in the comfort of their own homes.
Requirements:- Minimum 1 year RN experience preferably in acute, SNF, Home Health or Hospice setting
- Must possess an active CA Registered Nursing License
- AA, BSN, MSN degrees acceptable
- Reliable transportation
- Current CPR or BLS certification
