Requirements:

• Bachelor's Degree in Nursing (BSN) preferred
• Registered Nurse (RN) licensure in the state of practice required
• Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS) certification required
• Valid Driver's License and must be at least 21 years of age or older required
• Current automobile insurance and a reliable automobile required
• At least two years of recent experience in the field
• Home Health EMR experience required

Roles & Responsibilities:

• Plan, organize, and direct home care services to meet patient needs
• Manage a patient caseload effectively and efficiently
• Provide direct and/or indirect skilled nursing care, and coordinate patient care with other disciplines
• Complete initial and ongoing comprehensive assessments of patients' needs
• Document patient care delivery, patient status, and responses to treatment accurately
• Educate and counsel patients and families on healthcare and nursing needs
