Registered Nurse, Hospice Inpatient, PRN

• Provide hospice and palliative care to terminally ill patients and supportive care to their families in the Hospice House
• Assess patients' physiological, psychosocial, developmental, environmental, physical, emotional, spiritual, pain, and symptom needs
• Contribute to interdisciplinary Plans of Care and develop individualized nursing and CNA plans of care
• Educate patients and families and refer them to additional hospice services as needed
• Develop end-of-life plans of care according to patient and family wishes
• Perform nursing interventions and document assessments, interventions, orders, communications, referrals, and care-plan changes
• Report assessment findings and patient-condition changes to physicians and make recommendations
• Oversee CNA performance of the Plan of Care
• Evaluate intervention effectiveness, treatment and medication responses, disease progression, family coping, cultural needs, and developmental needs
• Support families through bereavement follow-up and related programs and services
• Coordinate continuity of care with hospice team members, physicians, agencies, pharmacy, DME vendors, material management, and insurance companies
• Initiate the Plan of Care at time of death, notify appropriate persons/agencies, and complete discharge paperwork
• Perform admissions, evaluation visits, and consultative services
• Participate in team meetings, projects, committees, public education, and orientation/training of students, new employees, and volunteers
• Perform other duties as assigned

• Associate degree in Nursing
• Current RN compact/multistate license recognized by the NCSBN Compact State, or RN licensure in the state where the team member works
• Knowledge of and compliance with licensure renewal, assigned training, employee health screenings, time and attendance, dress code, patient confidentiality, infection control, and medication administration requirements
• Knowledge of regulatory agency requirements, including The Joint Commission and DHEC
• Ability to provide care within the scope of the SC Nursing Practice Act
• Two years of acute care nursing preferred
• Experience in hospice or other home care services preferred
• BSN preferred

Demonstrates expertise in providing hospice and palliative care, including patient assessment, individualized care planning, and interdisciplinary collaboration. Proficient in regulatory compliance, nursing interventions, and bereavement support.
