• Collaborate with the team to develop, implement, and review personalized care plans for residents.
• Conduct thorough health assessments, identify medical concerns, and work with other professionals to create effective care plans.
• Administer medications as prescribed, ensuring accuracy, adherence to protocols, and monitoring effectiveness.
• Maintain accurate medical records, care plans, and incident reports to ensure regulatory compliance.
• Respond promptly to medical emergencies, providing immediate care and coordinating with emergency services when necessary.
• Communicate openly with residents’ families, keeping them informed about their health and changes.
• Advocate for residents’ rights and preferences, ensuring their voices are heard and respected.

• Valid and active RN or LPN license.
• Effective communication skills with residents, families, and healthcare professionals.
