SummaryThe Home Health and Hospice RN is responsible for client care, including direct nursing care, coordination of care, and coordination of services in the home for clients of all ages, to assist the client in achieving a maximum level of wellness and independence.
Essential Functions / Responsibilities- Coordinate client services and case management; provide direct skilled care and teach patient care skills to clients and/or caregivers.
- Initiate preventative and rehabilitative nursing procedures individualized to the client and age-specific needs.
- Develop and revise a plan of care based on initial assessment, ongoing reassessment, client history, nursing diagnosis, physician orders, expected outcomes, and discharge plans when appropriate.
- Utilize agency and community resources; advocate for clients and follow visit utilization guidelines and protocols.
- Formulate and implement teaching plans; communicate significant clinical findings and changes in status to the physician and other involved personnel.
- Document all skilled and medically necessary care thoroughly, timely, and accurately; ensure plans of care and documentation comply with Medicare, state and payer requirements as well as local policy.
- Maintain client and organizational confidentiality at all times; participate in QAPI (Quality Assurance and Performance Improvement) programs and required in-service trainings.
- Conduct Home Health Aide supervisory visits and prepare/update written instructions for Home Health Aides; ensure duties remain within the Nurse Practice Act.
- Collect patient health data and document findings using Meditech Home Care; complete OASIS time points accurately.
- Identify outcomes/goals and develop a plan of care with interventions to attain outcomes; collaborate with patients, families, and health care providers.
- Attend and actively participate in care conferences; contribute to the professional development of peers and colleagues.
- Make ethical decisions and consider safety, effectiveness, and cost in planning and delivering patient care.
- Assist with intake of referrals in home care; verify Medicare admission criteria and obtain benefits/preauthorization for commercial payers as indicated.

- Education/
Experience:
Diploma, Associate or Bachelor's degree from a college or university; or one to two years related experience and/or training; or an equivalent combination of education and experience. - Bi-lingual experience preferred but not required.
- Licenses/Certifications:
Current Registered Nurse license in the State of Iowa; CPR certification within 3 months of hire. - Other:
Access to an automobile; valid driver’s license and automobile liability insurance.
