Summary The Home Health and Hospice RN is responsible for client care to include direct nursing care, coordination of care, and coordination of services in the home for clients of all ages, unless otherwise specified, in order to assist the client in achieving a maximum level of wellness and independence.

General Hours of Work Rotating shifts with weekday/evening & weekend/holiday call involvement.

Education and/or Experience Diploma, Associates or Bachelor's degree from a college or university; or one to two years related experience and/or training; or equivalent combination of education and experience. Bi-lingual experience preferred but not required.

Certificate, Licenses, Registrations Current Registered Nurse license in the State of Iowa. Must obtain CPR certification within 3 months of hire date. Access to automobile required. This position requires a valid driver’s license and must carry their own automobile liability insurance.

Essential Functions Responsible for coordinating client services, case management, providing assigned clients with direct skilled care, and teaching patient care skills to the client and/or caregiver. Initiates preventative and rehabilitative nursing procedures individualized to the client and according to age specific needs. All duties will be performed within the scope of the Nurse Practice Act.

Develops and revises a plan of care based on the initial assessment, continual reassessment of client/family needs, client history, nursing diagnosis, physician orders, expected outcomes, and client goals to include discharge plans when appropriate. Utilize appropriate agency and community resources, be a client/family advocate and follow visit utilization guidelines and protocols. Formulates and implements appropriate teaching plans, communicates significant clinical findings and changes in status to the physician and other involved personnel, including the clients.
