Shifts- Full‑time and PRN
- Weekdays, weekends, evenings
- Urgent / Immediate
- G‑tube care
- Tracheostomy (trach) care

- Provide care within scope of practice as per licensure/certification
- Maintain updated clinical record for each patient at all times, meeting documentation deadlines
- Evaluate effectiveness of nursing service to patient and family on an ongoing basis
- Consult with physicians regarding alterations to patient care plans, document changes, and report all changes to nurse supervisor
- Mentor and coach new employees during patient‑specific orientations
- Educate patients and caregivers on care needs
- Participate in Quality Assurance, Performance Improvement (QAPI) as requested
- Continuously improve nursing care skills
- Attend and complete all mandatory training/in services as assigned
- Submit required personnel file documentation in a timely manner
- Adhere to compliance and ethics expectations
- Maintain patient and family confidentiality
- Maintain professional, supportive, and responsive interpersonal communication skills
- Perform other duties as assigned
- LPN/LVN Nursing Degree and unencumbered license or Multi‑State license via ENLC
- 1 year of demonstrated experience
- Valid driver license with proof of insurance (applicable states)
- Clear background screening
- Knowledge of Nurse Practice Acts
- Valid CPR card/certificate with in‑person instructor‑led hands‑on skills check
- Clear Hep B screening (applicable states)
- Clear TB screening (applicable states)
- Ability to read and communicate in English both verbally and in writing
- Work performed in patients' homes; not substantially exposed to adverse environmental conditions. Training may be performed in office setting.
- Must be able to lift up to fifty pounds (50 lbs) without assisted lifting device.
- Frequent use of office machines: telephone, computer, printer.
