Hours:

Full Time, 36 hours per week, 12 hour shifts

Schedule:

Self-Schedule, Days 7am-7pm or Nights 7pm-7am, rotating weekends or holidays

Provides direct, hands-on patient support across nursing units under the supervision of licensed staff. Responsibilities include assisting with activities of daily living, promoting safe ambulation and transport using appropriate equipment, accurately documenting care and treatments, monitoring patient safety and condition, and promptly communicating observations and changes to the care team.

Delivery of Care

• Assists patients with ADL’s according to hospital procedures, reports all abnormal findings and changes in patient condition tonursein charge of patient.
• Assists with ambulation of patients anduses assist devices as appropriate toprotectpatientand staff member.
• Assists with restocking of rooms andmaintaina safe and clean environment.
• Follows all established infection control policies andrecognizesrolein providing safety to the patient.
• Obtain EKG under the direction of the nurse.
• Documents vital signs, weights, activity, intake and outputs, and ADL care given topatientin computer.

Development

• Attends appropriate training programs, unit meetings and other mandatory meetings.
• Seeksto be involved in unit improvements, quality projects, and other enhancements to patient care.
• Effectively communicates with team members, physicians, ancillary staff,and customers. Verbal communication is clear and concise, and written communication is legible and done using approved abbreviations.
• Uses Patient/Family Centered approachofcare ofpatient/family.

Basic UPH Performance Criteria

• Demonstrates the UnityPoint Health Values and Standards of Behaviors as well as adheres to policies and procedures and safety guidelines.
• Demonstrates ability to meet business needs of department with regular, reliable attendance.
• Employeemaintainscurrent licenses and/or certificationsrequiredfor the position.
• Practices and reflects knowledge of HIPAA,DNV, OSHA and other federal/state regulatory agencies guiding healthcare.
• Completes all annual education and competency requirements within the calendar year.
• Is knowledgeable of hospital and department compliance requirements for federally funded healthcare programs (e.g.Medicare and Medicaid) regarding fraud,wasteand abuse.
  
  Brings any questions or concernsregardingcompliancetothe immediate attention of hospital administrative staff.
  
  Takes appropriate action on concerns reported by department staff related to compliance.

Education:

• Must be actively enrolled in high school or equivalent program.
• High school graduate or equivalent preferred.

Experience:

• Previous hospital or nursing home experience as a Patient Care Tech and/or completion of training in nursing school that covers Patient Care Tech type duties and/or must attend UPHSL Patient Care Tech Training.
• Previous hospital or nursing home experience preferred.

License(s)/Certification(s):

• Basic Life Support (BLS) within
  90 daysof hi rerequired.
• Certified Nursing Assistant preferred.
