The Registered Nurse (RN) Women's Health provides Veteran-centered care coordination and case management for Veterans with chronic and acute conditions. Works collaboratively with interdisciplinary teams, families, and Veterans to ensure continuity of care across all healthcare settings. Conducts post-discharge follow-up, assists with care planning, provides patient education, and advocates for Veterans while ensuring accurate electronic health record documentation.
* Collaborate with Veterans, family members, and interdisciplinary teams to promote positive clinical, financial, and psychosocial outcomes for Veterans with chronic and acute diagnoses
* Provide post-discharge follow-up contacts within two days for Primary Care patients to ensure understanding of medications, discharge instructions, and follow-up appointments
* Assist with post-hospital planning by determining appropriate community resources and evaluating patients' nursing care needs
* Integrate assessments of living conditions, family dynamics, and cultural background into patient care plans
* Serve as an advocate by supporting decisions, clarifying options, and problem-solving with the interdisciplinary team
* Ensure accuracy in electronic health record documentation and compliance with care coordination guidelines
* Participate in Performance Improvement activities to improve patient care through monitoring, analyzing, and evaluating care outcomes
* Apply critical thinking and analytical skills to identify barriers to Veteran care delivery and recommend improvements
* Communicate effectively with internal and external customers, including providers at the facility and Community-Based Outpatient Clinics (CBOC)
* Participate in educational activities to improve knowledge regarding all aspects of care management, clinical services, and professional development

Pay:
Competitive salary, regular salary increases, potential for performance awards
