In-Home Health - Nurse Practitioner or Physician Assistant; Per Diem - Sioux Falls, SD

About the Role

Signify Health, part of CVS Health, is seeking a Per Diem Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations. You will engage with members in the comfort of their own homes, helping bridge gaps in care and building trusted relationships to make people healthier.

The visit lasts about 45 minutes and includes a medication and medical history review, a physical evaluation, and, if ordered by the plan, additional diagnostic tests such as diabetic eye exams or spirometry. You’ll use an iPad and intuitive clinical workflow. You are not required to prescribe medicine, order lab tests, or alter current treatment regimens.

• Be flexible to travel locally and within licensed states as needed.
• Conduct virtual visits as required.
• Provide all Signify Health services indicated, including Diagnostic & Preventive Services products (spirometry, DEE, bone density screening, etc.).

Primary workplace: private residence of health plan members. Must comfortably and safely perform duties in homes possibly containing household pets, secondhand smoke or allergens.

Per diem employees are eligible for a base hourly rate plus incentives per visit completed. Signify Health covers malpractice insurance during related activities. Supplies and other perks are provided. The typical pay range is $37.66 – $81.11, based on experience, education, geography and other factors.

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Multi-state licenses (preferred).
