Home Hospice Nurse – Sioux Falls, South Dakota (Avera at Home Hospice)

Shift & Pay

Work Shift:

Day Shift. Pay Range: $31.00 – $46.25 per hour, depending on experience.

Job Overview

The Home Hospice Nurse provides comprehensive nursing care to Avera@Home patients, following ANA Standards of Clinical Nursing Practice. The nurse is responsible for patient outcomes in home or facility settings, ensuring high-quality, evidence‑based care.

Responsibilities
  • Collect patient health data.
  • Analyze assessment data to determine nursing diagnoses and problem focus.
  • Identify expected outcomes and goals individualized to the patient.
  • Develop a plan of care that prescribes interventions to attain expected outcomes.
  • Implement the interventions identified in the plan of care.
  • Evaluate the patient’s progress toward attainment of outcomes.
  • Systematically evaluate the quality and effectiveness of nursing practice.
  • Collaborate with patients, significant others, and health‑care providers to provide patient care.
  • Utilize research/evidence‑based findings in practice when appropriate.
  • Consider safety, effectiveness, and cost factors in planning and delivering care.
Essential Qualifications
  • Valid driver’s license; insurance coverage by Avera Health automobile liability insurance carrier.
  • Active Registered Nurse (RN) license in the state of practice.
  • Ability to perform essential job functions, including visual acuity, communication, hearing, and understanding of speech.
  • Capacity to work specified hours and shifts.
Preferred Qualifications
  • Minimum two (2) years of related experience or equivalent combination of education and experience.
