Position HighlightsHome Hospice Nurse for Sioux Falls. On call approximately 2 times a month. Service area is 40 mile radius of Sioux Falls and 30 mile radius of Madison and Flandreau.

A Brief OverviewResponsible for utilizing the nursing process to provide nursing care for Avera@Home patients following the ANA Standards of Clinical Nursing Practice. The Nurse assumes responsibility for the outcome of nursing care provided to patients in their homes or in facilities. The Nurse demonstrates the knowledge and skills necessary to provide exceptional care for the population served.
What you will do- Collect patient health data.
- Analyze the assessment data in determining nursing diagnoses/problem focus.
- Identify expected outcomes/goals individualized to the patient.
- Develop a plan of care that prescribes interventions to attain expected outcomes.
- Implement the interventions identified in the plan of care.
- Evaluate the patient’s progress toward attainment of the outcomes.
- Systemically evaluate the quality and effectiveness of nursing practice.
- Collaborate with the patient, significant others, and healthcare providers in providing patient care.
- Utilize research/evidence-based findings in practice if appropriate.
- Consider factors related to safety, effectiveness, cost in planning and delivering patient care.

- Drivers License - Licensing Board:
This position requires a valid driver’s license and that the employee is insurable by the Avera Health automobile liability insurance carrier upon hire. - Registered Nurse (RN) - Board of Nursing:
An active license in the state of practice upon hire.
- Minimum two (2) years related experience and/or training or equivalent combination of education and experience.
