Position Overview

The Registered Nurse (RN) – Hospital at Home provides acute‑level nursing care to patients receiving hospital services in their homes. This role delivers comprehensive, evidence‑based clinical care consistent with inpatient standards while promoting patient safety, comfort, and recovery in the home setting. The RN functions as part of an interdisciplinary care team and is responsible for assessment, clinical intervention, care coordination, and patient education within the Hospital at Home model.

  • Review physician prescriptions and communicate the nursing care plan to the team.
  • Coordinate with other healthcare providers and provide physical, emotional, psychological, and spiritual support to patients, families, and friends.
  • Develop nursing care plans based on medical history, examinations, and documented physical and psychological status.
  • Educate patients and families on conditions, medications, and self‑care skills.
  • Evaluate patient care against standards, adjust care objectives, and modify care plans as needed.
  • Maintain a safe, secure environment by following infection‑control policies, medication storage procedures, and controlled substance regulations.
  • Document patient care in charts and department records.
  • Ensure equipment operates properly; troubleshoot and maintain supplies.
  • Participate in ongoing competence development as outlined by leadership.

A Brief Overview

Responsible for care planning, coordination, and education within the interdisciplinary team. Develop patient care plans with the physician and team, ensuring efficient, effective patient‑centered care.

What you will do
  • Utilize clinical expertise to make recommendations to physicians based on comprehensive assessments.
  • Implement the nursing process during admission, post‑hospital evaluations, re‑certifications, discharges, and supervisory visits.
  • Partner with the physician and team to develop collaborative care plans.
  • Recommend efficient use of resources to achieve positive outcomes.
  • Respond to care‑plan changes and determine necessity of significant change documentation.
  • Educate patients, families, and caregivers on disease process and self‑care.
