Day Shift (United States of America). Pay range: $31.00 – $46.25. (Actual pay rate dependent upon experience.)
Home Hospice Nurse for Sioux Falls. On call approximately 2 times a month. Services are within a 40 mile radius of Sioux Falls and a 30 mile radius of Madison and Flandreau.
Brief OverviewResponsible for utilizing the nursing process to provide nursing care for Avera@Home patients following the ANA Standards of Clinical Nursing Practice. The Nurse assumes responsibility for the outcome of nursing care provided to patients in their homes or in facilities. The Nurse demonstrates the knowledge and skills necessary to provide exceptional care for the population served.
What You Will Do- Collect patient health data.
- Analyze assessment data to determine nursing diagnoses/problem focus.
- Identify expected outcomes/goals individualized to the patient.
- Develop a plan of care that prescribes interventions to attain expected outcomes.
- Implement the interventions identified in the plan of care.
- Evaluate the patient’s progress toward attainment of the outcomes.
- Systemically evaluate the quality and effectiveness of nursing practice.
- Collaborate with the patient, significant others, and healthcare providers in providing patient care.
- Utilize research/evidence‑based findings in practice when appropriate.
- Consider factors related to safety, effectiveness, cost in planning and delivering patient care.

RequiredEducation, License/Certification, or Work Experience
- Valid driver’s license (upon hire).
- Registered Nurse (RN) – active license in the state of practice (upon hire).
Minimum two (2) years related experience and/or training or equivalent combination of education and experience.
