A Brief OverviewProvides individual and group diabetes self-management education, conducts community and health professional education related to diabetes, and serves as a resource for the nursing staff relative to diabetes education and basic nursing care of the diabetes patient.
What You Will Do- Plans, conducts and evaluates system diabetes disease management processes and systems.
- Serves as a clinical resource person for providers and nursing staff related to the care and education of diabetes patients.
- Supports clinics in disease management and access to diabetes education.
- Identifies opportunities for program development and participates in special projects as assigned.
- Develops, conducts, and evaluates training program for clinic staff to provide diabetic education.
- Collaborates with diabetes management and education resources across the system.
- Maintains expert knowledge in diabetes education and case management tactics.

- Registered Nurse (RN) - Board of Nursing An active license in the state of practice Upon Hire
- 4-6 years of experience in disease management or case management
- Experience in disease management specific to diabetes mellitus
