Build and Lead a Case Management Team for a 40-Bed Rehabilitation Hospital A bit about us:We are a 40-bed Encompass Health rehabilitation hospital in Sioux Falls with all-private patient rooms and an average daily census of approximately 40.

The Director of Case Management will lead the hospital's Case Management department with a strong emphasis on discharge planning, utilization review, patient experience, and development of the Case Management team.
The department currently includes two full-time Case Managers and two contract Case Managers, with searches underway to add two additional full-time Case Managers. The DCM will have the opportunity to build this team while hospital leadership currently provides coverage for the Director function.
Key responsibilities will include:- Lead Case Management operations across the 40-bed hospital
- Manage, develop, and support the hospital's Case Management team
- Oversee discharge planning and utilization review processes
- Help improve patient experience and discharge-related performance
- Build a positive, accountable culture within the Case Management department
- Coordinate effectively with physicians, nursing, therapy, social work, and hospital leadership to support appropriate transitions of care
- Ensure Case Management practices align with hospital standards and applicable regulatory requirements
- Support Case Managers managing typical caseloads of approximately 12–13 patients
- Minimum 3 years of hospital-based Case Management experience
- Demonstrated Utilization Review and Discharge Planning experience
- Must be qualified to independently complete assessments within the scope of the candidate's professional discipline
- RN candidates must hold an active RN license and bachelor's degree in Nursing (BSN)
- RN candidates are strongly preferred
- Non-RN healthcare professionals must possess at least a bachelor's degree and applicable state licensure where available
- Social Work candidates must be licensed and bring relevant medical/hospital experience
- Graduate degree preferred for eligible non-nursing disciplines
- CCM or ACM certification required or willingness to obtain the applicable certification within the required timeframe
- Strong discharge planning experience is particularly important for candidates coming from hospital systems whose Case Management model differs from Encompass Health
