POSITION SUMMARY

The Case Manager for Veteran Services provides essential case management services and support to veterans and their families at risk of or currently experiencing homelessness. Coordinating closely with the Sioux Falls VA Homeless Case Managers and VA Liaison, the Case Manager helps veterans evaluate all available resources and ensures a comprehensive approach to care.

This role involves developing individualized action plans for each participant, holding regular meetings to assess progress, and gathering detailed information about the educational, financial, social, and emotional histories of the veterans and their families. Participation in advisory meetings with veterans and professional staff is required, as well as individual client meetings to address skills and progress.

Additionally, the role includes developing transition plans, providing aftercare and follow-up services, and maintaining strong relationships with referral sources to inform clients about the diversity of services available.

ESSENTIAL

FUNCTIONS STATEMENT(S) Program Development

A. Promote and coordinate development of support services between agency, Veterans Administration, and community.

B. Develop and maintain a working relationship with referral sources; provide information about diversity of services available.

C. Assist the Program Coordinator in making contact with community organizations and resources to engage the participation of homeless veterans.

D. Act as liaison with property managers to facilitate housing leases and resolve any issues with program participants.

Program Management

A. Ensure consistent and ongoing communication with immediate supervisor for program monitoring purposes.

B. Complete timely and thorough documentation of participant services and activities including assessments, intakes, progress reports, and monthly progress reports.

C. Submit billing information and census data for review by the coordinator no later than the second working day of the month.

D. Provide a report on veteran and family progress in groups and support services to Veteran Services Team.

E. Attend training required to meet program

F. Ensure that participant files are in compliance with all state, federal, and accreditation

G. Conduct inspections in collaboration with the VA on new apartments and current participants to insure safety and health compliance.

Carry out other duties as requested due to program modification or expansion.

POSITION QUALIFICATIONS SKILLS & ABILITIES

Education: High school diploma or GED required. Associates Degree in social work or related human services field preferred.

Experience: Minimum of two years related work experience is preferred, experience working with veterans or family support services is preferred. Military service or experience preferred.

Skills: Excellent written and verbal communication skills. Ability to manage multiple priorities and balance perspectives. Effective communication, appropriate boundaries, flexibility and willingness to learn are essential.

PHYSICAL DEMANDS

A. Ability to Stand, Walk, sit, manually manipulate, reach outward and above shoulders, speak, climb, crawl, squat, kneel, and bend on a daily basis

B. Capability to Lift, carry, push or pull up to 25 lbs on a daily basis
