Job Overview

• Conduct psychosocial assessments to identify patients’ emotional, social, and financial needs.
• Develop and implement care plans addressing patient concerns.
• Provide counseling and crisis intervention for patients and families.
• Advocate for patient rights and assist with healthcare decision‑making.
• Coordinate discharge planning and transition of care services.
• Educate patients and families about community resources and support programs.
• Maintain detailed case records and ensure compliance with healthcare regulations.
• Collaborate with healthcare providers to ensure holistic patient care.

• Master’s degree in Social Work (MSW).
• Licensed Clinical Social Worker (LCSW) or equivalent state licensure.
• Additional certifications in healthcare social work preferred.
• 1+ years of experience in medical or healthcare social work.
• Strong advocacy and problem‑solving abilities.
• Excellent communication and counseling skills.
• Knowledge of healthcare systems and patient rights.
• Willingness to relocate temporarily for assignments.
