Pay Range: $36.46 - $51.29 /hour (equivalent to $82,000 - $106,000 annually). Sign-on and relocation bonuses up to $35,000 are available.
We are hiring a Social Services Clinician for our LevelIV Critical Access Hospital in Southeast Alaska. This role supports patients from birth through elder, with a focus on discharge planning, care coordination, and cultural wellness.
What you will do- Conduct clinical assessment and brief therapy for patients of all ages.
- Facilitate patient care conferences and lead discharge planning for Acute Care Facility (ALF) or transitional placements.
- Provide clinical support for chemical dependency, mental health needs, and appropriate referrals.
- Work closely with hospital teams and local community partners to ensure optimal service utilization.
- Advocate for clients, build skills, and assist with complex matters such as Power of Attorney.
- Ensure smooth transitions of care throughout the continuum.
- Mention accurate, timely, and compliant documentation in alignment with state, federal, accreditation, and billing standards.
- Complete screenings, assessments, treatment plans, progress notes, and required reports on time.
- Master’s Degree in Social Work, Psychology, Counseling, or a related discipline from an accredited institution.
- Licensed as or in the process of becoming a Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC) or Licensed Marriage & Family Therapist (LMFT). The license must be valid in the state of Alaska.
- Valid Alaska driver’s license, or ability to obtain one within six months of hire.
- Must be 21 years of age or older.
- Clinical competency required within three months of hire and maintained every three years.
- Demonstrated knowledge of discharge planning interviewing techniques, crisis intervention procedures, ethical and legal management practices, patient legal rights, and documentation regulations specific to accreditation requirements.
- Proficient in performing case management, using tact, discretion, initiative, and independent judgment within established guidelines.
- Ability to observe, record psychiatric behavior, and substance use; conduct screenings, assessments, treatment planning, and documentation in progress notes.
- Ability to analyze, evaluate, and diagnose situations and adopt effective courses of action.
- Skill in writing clear and concise reports and chart notes in accordance with professional standards, state regulations, and department protocols.
- Clinical Competency Assessment – SEARHC.
- Position covered under the Indian Child Protection Act (ICPA).
- Maintain complete and accurate records (paper and electronic), case notes, statistics, and other documentation in a confidential manner according to department policy.
- Ensure documentation meets all hospital billing and accreditation standards.
- Coordinate with corporate and community social service resources to ensure appropriate service utilization.
- Participate in multidisciplinary team meetings, assist in developing and monitoring treatment plans, and counsel other professionals as needed.
- Assist with scheduled staff trainings/meetings, and help staff integrate new information/practices into delivery of client services.
- Attend SEARHC Quality Management trainings and incorporate concepts and skills into client services.
- Complete all billing duties assigned by lead program clinical personnel and complete provider enrollment documentation on time.
