South East Alaska Regional Health Consortium seeks a Social Worker to join the Long-Term Care interdisciplinary team. You will interview residents and families, assess psycho-social needs, and help develop discharge plans across LTC, inpatient, or outpatient settings.
You will collaborate with medical providers, finance staff, and community resources to maximize payer reimbursement and coordinate transportation and services, always championing patient advocacy and confidentiality.
