Job Summary:

As a Travel Registered Nurse (RN) - Care Manager, you will be responsible for coordinating and overseeing the care of patients in various healthcare settings. You will work closely with physicians, nurses, and other healthcare professionals to ensure that patients receive high-quality and comprehensive care.

Responsibilities and Duties:

Assessing patient needs and developing individualized care plans

Coordinating and managing patient care across various healthcare providers

Monitoring patient progress and adjusting care plans as needed

Educating patients and their families on health maintenance and disease prevention

Advocating for patients' needs and rights

Collaborating with healthcare team members to ensure optimal patient outcomes

Qualifications and Skills:

Valid RN license

Minimum of 2 years of experience as a registered nurse

BSN or equivalent degree in nursing

Ability to work effectively in a fast-paced and dynamic healthcare environment
