Job Summary:
The Nurse Care Coordinator manages patient care plans, ensures appropriate utilization of healthcare resources, and collaborates with healthcare teams to promote quality outcomes. This role involves conducting chart reviews, assessing patient progress, and facilitating effective communication among providers, payers, and patients.
Responsibilities:
- Evaluate patient records and monitor patient progress to ensure optimal care delivery.
- Review hospital admissions and develop personalized treatment and discharge plans.
- Collaborate with physicians, clinical teams, and leadership to maintain compliance with regulatory and payer requirements.
- Validate physician orders, review medical reports, and ensure documentation supports utilization review functions.
- Coordinate patient care activities, provide educational resources, and communicate healthcare benefits to patients and families.
- Manage appeals and denials, maintain logs, and prepare required documentation for regulatory and payer agencies.
Qualifications:
- Active nursing license in Alaska required; must be available at the time of submission.
- Bachelor's Degree in Nursing or related field required.
- 8 years of clinical care or nursing experience, with at least 3 years in chart review, risk management, or related quality services.
- Proficient in medical terminology, ICD-9 and CPT coding, and CMS regulations.
- Strong communication, organizational, and collaborative skills.
- Case Management certification by a recognized organization (e.g., NCQA, CCMC) preferred.
