Care Manager, Registered Nurse

The Ambulatory Registered Nurse (RN) provides comprehensive, patient-centered nursing care in an outpatient clinic setting. This role supports individuals and families across the continuum of care. The position utilizes the nursing process to assess, plan, implement, and evaluate care. Responsibilities include administering medications and treatments according to provider orders and protocols. The RN performs clinical procedures and provides patient education and care coordination in collaboration with providers and interdisciplinary teams.

The Ambulatory RN ensures accurate documentation and adherence to infection control and regulatory standards. Efficient clinic operations are promoted while maintaining quality outcomes and patient safety. Culturally responsive care is delivered in a fast-paced ambulatory environment. Required competencies include:
Performing comprehensive patient assessments and developing individualized plans of care. Implementing and evaluating nursing interventions and patient responses. Recognizing changes in patient condition and initiating appropriate interventions or escalation of care. Delivering effective patient and family education related to care, medications, and discharge needs. Exercising independent nursing judgment in determining appropriate nursing actions and interventions. Developing, implementing, evaluating, and revising individualized nursing plans based on patient needs and clinical circumstances.

Integrating clinical findings with ethical principles, cultural considerations, and patient safety standards. Accepting accountability for nursing decisions, delegated tasks, and outcomes of nursing care. The role requires collaboration with providers and interdisciplinary team members to coordinate care. Facilitation of referrals, follow-up care, and continuity of services is essential. Accurate and timely documentation of assessments, interventions, and outcomes in the electronic health record is crucial.

Advocating for patient care and assisting patients in navigating the healthcare system is part of the role. The RN practices within their scope of practice and adheres to regulatory, safety, and infection prevention requirements. Participation in quality and performance improvement activities is expected. Maintenance of required competencies and certifications is mandatory. Reporting adverse events, near misses, and unsafe conditions according to policy is essential.

Professionalism, accountability, flexibility, and teamwork are vital attributes. Providing appropriate guidance and delegation to ancillary staff is necessary. Support for orientation, mentoring, and education of staff and peers is encouraged. Floating to other departments or assignments may be required.
