MSW Social Worker – Transitional Care

10/5/2026 Pay Rate: $1600.00 - $2300.00

POSITION DUTIES – Provides proactive social work and care management services for patients across the lifespan, collaborating with the multidisciplinary care team to address psychosocial needs, discharge planning, barriers to care, financial and housing concerns, placement coordination, custodial issues, and behavioural health referrals. Assists patients and families with supportive counseling, resource linkage, and access to community services, while managing swing bed admission assessments and patient disposition planning.
