Exciting opportunity to join Endeavor Health! The Department of Orthopedics is currently looking for a highly motivated Advanced Practice Provider (APRN/PA) to lead a patient‑facing, clinically intensive program focused on Orthopaedic trauma care for geriatric hip fracture patients and outpatient total joints. This role is central to coordinating interdisciplinary care, timely discharges, reducing patient and family anxiety, encouraging recovery at home whenever possible, and driving improved outcomes through collaboration, data analysis, and effective communication.

    Full‑time (average 40 hours/week)
  • Hours:

    5 8‑hour days Monday through Friday 9 a.m.–5:30 p.m. (no weekends/holidays/call)
  • Required Travel: n/a
  • Hourly Pay Range: $64.89 – $90.85 per hour (dependent on expertise and experience)
Clinical & Patient‑Facing Care:
Trauma
  • Lead Home Run Rounds daily, coordinating with Nursing, PT, Social Work, and Physician Assistants, dedicating 1–1.5 hours per day to interdisciplinary rounding on ~10–15 geriatric hip fracture patients.
  • Serve as the patient experience owner by connecting directly with patients and families, reducing fear and anxiety related to discharge planning, and reinforcing the philosophy that “home is best” for recovery when safe and appropriate.
  • Provide consistent clinical follow‑up and bedside presence to ensure continuity of care, clarity of plan, and patient/family confidence.
Clinical & Patient‑Facing Care:
Total Joint Arthroplasty
  • Post‑op rounding on ~10–15 outpatient total joint patients scheduled for same‑day discharge, including acute post‑op symptom review and management, physical exam and home recovery expectations.
  • Care coordination with nursing, physical therapy and discharge planners for safe and timely discharge.
Interdisciplinary Collaboration & Team Building
  • Foster a collaborative environment by engaging Nursing, PT, Social Work, Case Management, and physician colleagues in shared decision‑making.
  • Actively build team cohesion across departments to strengthen communication, trust, and accountability.
  • Coordinate comprehensive care plans across inpatient and post‑acute settings, ensuring smooth transitions for patients.
  • Conduct SNF surveillance to decrease length of stay, improve patient outcomes, and coordinate closely with providers at tertiary care centers.
Leadership & Program Development
  • Serve as the clinical leader of the program with ownership of day‑to‑day operations and growth.
  • Demonstrate strong decision‑making skills, interpersonal effectiveness, and the ability to serve as the liaison between patients, families, and the interdisciplinary care team.
  • Hire, mentor, and collaborate with additional APPs as the program expands.
Data, Analytics, & Outcomes
  • Track data related to outcomes, patient experience, LOS, and post‑acute utilization.
  • Continuously identify opportunities to enhance patient experience and operational efficiency.
Qualifications
  • License:
    Advanced Practice Licensure (APN) or Physician Assistant (PA) Licensure in the State of IL;
    Federal DEA and Illinois Controlled Substance License required.
  • Education:

    Completion of master’s level program in a specialty area.
  • Certification: BLS/CPR certification required.
  • Experience:

    Minimum 3 years of clinical experience, preferably in trauma, orthopedics, or acute care.
Special Knowledge and Skills Required
  • Experience with care transitions, SNF or post‑acute care coordination, and interdisciplinary rounding.
  • Strong data analysis, presentation, and outcome measurement skills.
  • Demonstrated success in patient‑facing roles, with proven ability to reduce patient/family anxiety and communicate complex care plans in understandable language.
  • Strong interpersonal and team‑building skills with the ability to foster collaboration across multiple disciplines.
