RN Field Coordinator (Korean Speaking)
Join Longevity Health and make a difference in long‑term care. This role supports continuity of care data gathering, onboarding new members, and training new nursing facilities.
About the RoleAs RN Field Coordinator, you will work with local clinical staff, educate nursing facilities on our clinical model, analyze EMR data, and communicate needs to the National Care Coordination Team. You will facilitate initial visits for NPs, travel to nursing homes, and collaborate with the Market Director of Clinical Services.
Key Responsibilities- Work with clinical and sales teams to identify member external appointments and facilitate access to providers.
- Use facility EMRs to prescreen new enrollees and determine continuity of care needs.
- Perform care coordination activities, including member transitions, prior authorization requests, and communication with families and doctors.
- Participate in Longevity Health Plan NC standing clinical meetings on a daily/weekly/monthly basis.
- Gather data for hospitalization review sessions.
- Onboard new nursing homes, educating team members on the LHP model.
- Assist with the initiation of telehealth services in newly onboarded homes.
- Act as a proficient user of the LHP EMR.
- Conduct nursing home orientation sessions and provide EMR data input support as requested.
- Perform other duties as assigned based on caseload and workload.

Credentials & Coverage- Registered Nurse licensed in good standing.
- Valid state driver’s license with a good driving record and proof of automobile insurance.
- Proof of insurance coverage prior to hire and ongoing.
- Associate degree in nursing required; bachelor’s degree preferred.
- Licensed professional registered nurse.
- Korean language proficiency heavily preferred.
- Background in nursing homes or health plan case management preferred.
- Minimum two years of experience as a registered nurse.
- Excellent verbal and written communication skills.
- Proficient computer skills, EMR/clinical platform, MS Office.
- Knowledge of challenges and opportunities in long‑term nursing home care.
- Willingness to travel.
- Managed care coordination experience required.
