Job Summary: We are seeking a full‑time Nurse Practitioner to provide psychiatric evaluations, medication management, and follow‑up care for residents in nursing homes and assisted living settings. You’ll be supported by a full‑time medical director, administrative staff, and outsourced billing services, so you can focus on patient care.
Responsibilities:
- Conduct initial psychiatric assessments and routine follow‑up visits
- Prescribe and manage psychotropic medications
- Collaborate with facility staff, medical directors, and care teams
- Document in EMR in a timely and accurate manner

Nurse Practitioner
Job Qualifications:
- Must have a current and active NP license and DEA # in the State of Illinois.
- Must have at least one year experience working as a Nurse Practitioner.
- Be willing and able to commute to long term care facilities.
- Experience with geriatric populations or interest in working with older adults
- Compassionate, patient‑centered care philosophy
- Strong communication and collaboration skills
Job Type: Full‑time
Pay: Competitive base + productivity bonus (commensurate with experience)
Schedule: Monday to Friday (flexible hours)
Work Setting: On‑site at skilled nursing/long‑term care facilities
