Home And Community Based Services Licensed Practical Nurse

Provide direct nursing care to acutely and chronically ill patients in their homes, following physician prescriptions. Collaborate with the healthcare team, document patient visits thoroughly, and participate in care coordination activities to ensure high-quality patient care. Work under supervision of a Registered Nurse or designated supervisor.

• Deliver clinical services within the scope of practice, adhering to state laws and infection control procedures.
• Accurately document patient visits in electronic medical records and visit verification systems.
• Participate in care coordination by reporting patient needs and condition changes, attending case conferences, and communicating effectively with patients, families, and healthcare providers.

• Current LPN licensure and CPR certification.
• Valid driver’s license, vehicle insurance, or reliable transportation.
• Ability to work effectively in diverse home settings and manage multiple tasks independently.
• Strong communication, organizational, and documentation skills.
• Preferably, home care experience and flexibility to meet client needs.
