, the CNA Transportation Coordinator provides compassionate, hands-on resident care while coordinating medical appointments and transportation services to ensure residents receive timely access to healthcare.
Schedule and coordinate physician appointments and other medically necessary resident appointments.
Arrange transportation for residents to and from medical appointments while ensuring schedules are accurate and communicated appropriately.
Coordinate with transportation providers, families, and healthcare offices to facilitate seamless resident care.
Maintain accurate records of appointments, transportation schedules, and related documentation.
Provide basic resident care under the direction and supervision of licensed nursing staff in a safe, compassionate, and respectful manner.
Assist residents with activities of daily living, including feeding, bathing, dressing, grooming, mobility assistance, and changing linens.
Provide supportive care to ensure residents' physical, emotional, and daily living needs are consistently met.
Observe residents and promptly report changes in condition, behavior, or care needs to nursing staff.
Maintain a clean, safe, and comfortable environment that promotes resident dignity and well-being.
Support nursing staff and the interdisciplinary team in delivering high-quality, resident-centered care.
Must hold a current, active, and unencumbered Certified Nursing Assistant (CNA) license in the state of Rhode Island.
Valid Basic Life Support (BLS) / CPR certification required.
Previous experience scheduling medical appointments and coordinating resident transportation is required.
Ability to work effectively with multiple residents in a fast-paced, team-oriented healthcare environment.
Demonstrated compassion, patience, and commitment to providing high-quality resident care.
Strong communication, organizational, and scheduling skills.
Ability to coordinate multiple appointments while maintaining accuracy and attention to detail.
