Job Summary:
We are seeking a dedicated and compassionate Licensed Practical Nurse Assessment Coordinator (LNAC and MDS) to join our team at Evergreen Post Acute Rehab in beautiful Smyrna, DE. As a valued member of our team, you will play a critical role in the assessment and care of our patients, ensuring they receive the highest quality care and attention. Responsibilities:
* Process related to the assessment of patients and residents in a healthcare facility
* Assess patients, asking patients to report how they feel, providing regular reports to doctors and other medical professionals, billing, and working with the MDS (medical decision support) system
* Assist with patient's examinations and/or questions
* Implement and interpret organizational policies and procedures, and nursing standards and regulations
* Assist with preparing the departmental fiscal budget
