Job Summary

We are seeking a dedicated and compassionate Licensed Practical Nurse Assessment Coordinator (LNAC and MDS) to join our team at Evergreen Post Acute Rehab in beautiful Smyrna, DE. Responsibilities
  • Process related to the assessment of patients and residents in a healthcare facility
  • Assess patients, asking patients to report how they feel, providing regular reports to doctors and other medical professionals, billing, and working with the MDS (medical decision support) system
  • Assist with patient's examinations and/or questions
  • Implement and interpret organizational policies and procedures, and nursing standards and regulations
  • Assist with preparing the departmental fiscal budget
