Work Shift:
Day (United States of America)
The LPN works under the direction of a Registered Nurse or Manager and functions within the LPN Scope of Practice. The LPN is responsible for providing direct patient care to patients with medical/surgical needs, including age‑appropriate interventions that meet physical, emotional, developmental, psychosocial and educational needs. The LPN coordinates and delivers high‑quality care, participates in call rotation and weekend visits as needed, and can serve as a preceptor for new LPN employees.
Attendance at staff meetings, in‑services, special projects and continued education is required.
- Direct Patient Care
- Assess and assist with managing patients' total needs following physician orders with RN oversight and using the nursing process.
- Perform routine nursing procedures to meet patient needs under RN supervision.
- Meet established productivity standards.
- Coordination of Services for Episode of Care
- Coordinate care and services with RN and other team members.
- Inform RN, therapist, and/or clinical supervisor of changes in patient condition, as appropriate.
- Collaborate with RN to develop and revise individual plans of care based on patient need and physician orders.
- Perform other duties as assigned by Home Health leadership.
- Patient / Caregiver Education
- Serve as patient educator to foster self‑care or caregiver support for the home patient.
- Be aware of and report concerns to authorities or agency leadership on any issues that affect patient or staff safety.
- Communication and Documentation
- Responsibly complete required documentation within established time frames.
- Demonstrate knowledge and appropriate application of regulatory guidelines.
- Participate in care team conferences with co‑workers.
- Check voicemail box and secure messaging at least once daily.
- Staff Education and Professional Development
- Participate in orientation and education of new employees and nursing students as appropriate.
- Attend continuing education and in‑services as directed and needed.
- Remain current in clinical knowledge for the care of the home health patient.
- Perform other duties as assigned and comply with all Wellstar Health System policies, standards of work, and code of conduct.
Minimum Education
- High School Diploma, General or GED
- General or Other / Certificate Nursing
- BLS
- Basic Life Support - ARC‑BLS
- American Red Cross Basic Life Support - BLS‑I
- Basic Life Support
- Instructor - LPN
- Licensed Practical Nurse
- All certifications are required upon hire unless otherwise stated.
- Minimum 1 year medical/surgical nursing practice in an acute care environment.
- Preferred:
Additional relevant experience.
- Clinical nursing skills aligned with the LPN scope.
- Effective communication and documentation abilities.
- Ability to collaborate with interdisciplinary teams.
- Commitment to ongoing professional development.
