Care Home Health is seeking a compassionate Licensed Practical Nurse (LPN) to provide skilled nursing care to an adult and geriatric patient population throughout the Dover, Frederika, and Townsend, DE communities.

• Full Time 40 hours a week
• Day shift
• One weekend/month
• Two holidays/year
• Two on-call shifts/month

• Provide skilled nursing care and prescribed treatments in the home setting.
• Care for adult and geriatric patients with a variety of healthcare needs.
• Develop and implement patient-centered care plans.
• Collaborate with physicians, caregivers, and interdisciplinary teams.

• Current Delaware LPN license or Compact State license.
• Minimum 1 year of LPN experience.
• Home Health and/or Med‑Surg experience preferred.

Hourly Pay Range: $25.52 - $38.28
