Role:
Hospice Admission RN, a licensed registered nurse responsible for educating patients and families about hospice services, performing assessments, and coordinating admission processes.
Responsibilities:Educate families on hospice care and eligibility criteria, obtain verbal certification and admission orders, conduct comprehensive assessments, review medical records and medications, develop individualized care plans, coordinate resources for patient safety, provide emotional support, and collaborate with interdisciplinary team members.
Qualifications & Skills:Associate's or Bachelor's Nursing degree, valid RN license (single or multi-state), BLS certification, valid driver's license, and private vehicle insurance. Requires minimum 2 years of acute care and hospice experience, with knowledge of Medicare/Medicaid benefits, patient advocacy, and emotional support.
Key Duties:Ensure compliance with policies, uphold ethical nursing standards, support patient-centered care, and facilitate smooth hospice admission and planning processes.
