We are looking for an RN to join our Home Health team. Come make a difference in the Smyrna, TN area and surrounding communities!

The RN provides and directs provisions of nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies. The role includes creating, implementing, executing, and coordinating the overall plan of care to promote wellness and prevent re-hospitalization for patients in their homes; maintaining continuity of patient care by planning and exchanging information with physicians, agency personnel, patients, families, and community resources;

making the initial and/or comprehensive nursing evaluation visit; determining the primary focus of care; developing the plan of care within state-specific guidelines; ensuring patients meet home health eligibility and medical necessity guidelines as defined by the payer; maintaining and submitting accurate documentation and clinical notes, including the patient’s condition, appropriate goals, interventions, and patient’s response to home health care; establishing and maintaining an effective and efficient visit route;

and providing high quality clinical services within the scope of practice as defined by state laws from admission through discharge.

• One (1) year of clinical RN experience.
• Current RN license in the state where services are rendered.
• Current CPR certification, driver’s license, vehicle insurance, and access to a dependable vehicle or public transportation.
• Must be able to reliably commute to multiple patient residences daily within the geographic area assigned.
