Position Summary
Clinical Manager / Clinical Team Lead responsible for ensuring patient care is coordinated and managed appropriately, overseeing clinical personnel and supervising services.
Essential Responsibilities and Duties- Develop and initiate the plan of care and conduct ongoing interdisciplinary assessment.
- Coordinate preventative and rehabilitative nursing procedures as ordered.
- Ensure a Clinical Manager/Supervisor is available during all operating hours.
- Develop, implement, and update individualized plans of care and communicate with physicians.
- Receive and evaluate case referrals, conduct home visits, determine home health needs.
- Assign appropriate home care personnel.
- Attend conferences with physicians regarding service eligibility.
- Evaluate case outcomes, assess clinician performance, provide guidance.
- Review patient medical diagnoses, procedures, medications, and clinical course.
- Assist clinicians in establishing therapeutic goals and priorities.
- Lead and document case conference meetings.
- Participate in quarterly record reviews and report findings.
- Assist in screening, interviewing, and orienting new personnel.
- Help plan, implement, and evaluate in‑service education programs.
- Participate in public relation and community activities.
- Participate in quality assessment and performance improvement.
- Perform other duties delegated by Director of Nursing or Branch Manager.
- Current licensure as an RN, Physician, PT, OT, SLP, or Social Worker in the state of practice.
- Minimum two (2) years of supervisory/management experience in home health or related setting (preferred).
- Current CPR certification.
- Valid Driver’s License, vehicle insurance, and reliable vehicle or public transportation.
- Excellent time‑management, observational, and problem‑solving skills.
- Outstanding verbal and written communication skills.
- Self‑motivated, capable of working independently and collaboratively within an interdisciplinary team.
May supervise employees performing similar or related work in the same department.
Physical Demands and Work EnvironmentHeavy use of office equipment, standing, walking, sitting, and handling up to 20 pounds. Over 50% of duties require travel to client locations via car or public transportation. Work schedule may include evening, night, holiday, or weekend assignments.
