Role Overview:
The Home and Community Based Services Licensed Practical Nurse provides direct nursing care to acutely and chronically ill patients in their homes, under supervision.
Key Responsibilities:- Deliver high-quality clinical services within legal scope, following care plans and infection control protocols.
- Accurately document patient visits in electronic medical records and visit verification systems.
- Participate in care coordination by reporting patient needs, attending case conferences, and communicating with patients, families, and healthcare team members.
- Adapt to diverse home environments, respecting patient diversity.
- Current, unrestricted LPN licensure in the relevant state.
- Valid driver's license, vehicle insurance, or reliable transportation.
- Preferred: CPR certification, flexibility, multitasking ability, independence, and strong communication skills.
The role offers competitive hourly pay, benefits, and incentives, with pay ranging from $22.90 to $34.35 per hour. The position emphasizes a culture of caring, connecting, and growth, supporting professional development and inclusive practice.
