Blue Summit Hospice and Palliative Care in Georgia is seeking a Hospice Admissions Registered Nurse (RN) to assess patients for hospice eligibility and coordinate admissions with the interdisciplinary team.
You will educate families, develop individualized care plans, and ensure timely, compassionate admission processes. The role requires RN licensure, at least 2 years of nursing experience (prefer hospice/palliative care), and strong communication; CHPN and EMR experience are preferred.
