JOB TITLE:

Family Medicine Nurse Practitioner

POSITION SUMMARY:

The Family Medicine Nurse Practitioner (NP) provides comprehensive, patient-centered primary care to patients across the lifespan. The NP is responsible for assessing, diagnosing, treating, and managing acute and chronic health conditions while emphasizing preventive care, health education, and wellness. This position partners with patients, families, and the interdisciplinary care team to develop individualized care plans, promote healthy outcomes, and ensure continuity of care.

ESSENTIAL FUNCTIONS:

Maintain accurate, timely, and confidential medical records in accordance with organizational and regulatory requirements.

Provide evidence-based psychiatric evaluation, diagnosis, treatment, and medication management within the scope of practice.

Monitor medication effectiveness, adherence, side effects, potential interactions, and overall treatment response.

Maintain current knowledge of behavioral health best practices, psychiatric medications, clinical guidelines, and regulatory requirements.

Participate in quality improvement initiatives, clinical case reviews, and interdisciplinary team meetings.

Maintain complete and timely documentation in the electronic health record in accordance with organizational and regulatory standards.

Identify opportunities to improve patient outcomes, access to care, clinical workflows, and the overall patient experience.
