• Evaluate, plan, and deliver skilled physical therapy services to SNF residents
• Create individualized treatment plans that improve mobility and quality of life
• Document patient progress clearly and timely in accordance with facility and CRG policies
• Collaborate with nursing and interdisciplinary teams to coordinate care
• Coach patients and families on home programs and safe mobility strategies
• Meet productivity standards and contribute to a high-performing therapy department
• Adhere to dress code and facility procedures to maintain a professional presence

• Current Physical Therapist license in New Mexico (active and in good standing)
• DPT preferred; PT degree required
• 1-3 years of clinical experience, preferably in a skilled nursing facility
• Current BLS/CPR certification
• Strong documentation skills and familiarity with productivity expectations (90% productivity requirement)
• Reliable transportation and ability to be onsite to receive guaranteed hours

• Doctor of Physical Therapy (DPT) preferred;
  Physical Therapy degree required

• Skilled in SNF assessment and treatment planning
• Proficient with patient transfers, gait training, and therapeutic exercise
• Collaborating with interdisciplinary teams
• Good time management and organizational skills for productivity targets
• Strong interpersonal skills for coaching patients and working with families
• Familiarity with facility policies and regulatory documentation requirements
