Provides patient care in the ambulatory care setting according to established standards of care, in collaboration with Care Managers and Medical Providers as members of the primary care team.

• Adheres to the LPN scope of practice
• Administer prescribed medications and treatments, as ordered by provider/Standard of Care Protocol.
• Provides patient care in the ambulatory care setting according to established standards in collaboration with Providers, Registered Nurses and Medical Assistants as members of the care team.
• Provides primary support to the provider by proactively anticipating the provider needs, preparing the patient for the visit, maintaining efficient patient flow, and assisting with treatments and procedures.
• Participates on interdisciplinary team to meet patient care and clinic needs, working cooperatively with others to accomplish required work in a timely and efficient manner. Assists providers with procedures.
• Assists with patient education, within scope of practice by Standard of Care-Education Protocol.
• Assists with visit planning, within Health Maintenance Guidelines.
• Assist with management, in collaboration with Clinical Manager.
• Documents in electronic medical record within scope of practice.
• Assists in screening of patients, in collaboration with Care Manager.
• Assists Certified Medical Assistants with patient preparation/rooming patients, as directed by visit plan.
• Applies Compassionate Connected Care principles and incorporates into care interactions
• Interacts effectively with customers, providing instructions and information, both written and verbal, as appropriate.
• Maintains established practice policies and procedures, objectives, quality program, safety, environment, and infection control standards.
• Seeks help, communicates, and cooperates with Medical Provider and team appropriately.
• Meets assigned productivity targets
• Performs other functions as required.

• High School Diploma or GED
• Current Licensed Practical Nurse-NM
• BLS required
