Responsible for utilizing the nursing process to provide direct patient care to an assigned group of patients during a shift. Responsible for prioritizing the delivery of direct nursing care using time and resources efficiently integrating a standards-based framework model.

Work Shift:

10 Hour Days (United States of America).

• Acts as clinical resource by overseeing the delivery of patient care
  , directs and delegates patient care assignments to home health aides and nursing staff based on acuteness of patient care needs and staff capability and anticipates future needs
• Maintains clinically competent in providing direct patient care
  , evaluates outcomes, consults with other specialists as required and adjusts nursing care processes as indicated to ensure optimal patient care.
• Coordinates patient care through collaboration with the patient, family, all other disciplines, primary and secondary physicians and community resources/programs.
• Demonstrates knowledge philosophy and provides support to patients and their families.
• Ensures that patients understand their rights and responsibilities, advanced directives and the meaning of informed consent during the admission process.
• Assesses and directs patient and family teaching needs and is proficient in the use of return demonstration and teaching aids in the home setting.
• Maintains professional responsibility for knowledge of current nursing research, practice standards, guidelines and protocols, and hospital policies, procedures, and programs to improve patient care quality.
• Maintains a proficiency in the use of all equipment used in his/her practice.
• Effectively communicates and assists other members of nursing team and/or other disciplines. Actively participates in problem solving and conflict resolution to maintain team cohesiveness.
• Performs on-call duties effectively with the ability to triage patients over the phone and visit when needed.
• Directs own educational needs for the top 10 ICD9 codes and/or diagnoses for diabetes, wounds/post-surgical, arthritis, newborns, Coumadin dependency, malnutrition/ nutrition needs, medication instruction, pain and symptom control/hospice care.
• Supervises the home health aides in compliance with the Medicare/Medicaid rules and regulations.
• Performs other functions as required.

• Degree from accredited academic nursing program
• Minimum ADN preparation
• Licensure:
  State of New Mexico or Compact State Nursing License required.
• Required Certification from either AHA, ASHI, or Red Cross
• BLS:
  All Registered Nurse roles
• ACLS:
  Emergency Department, Critical Care, Cardiovascular Lab, GI Lab, Interventional Radiology, Progressive Care, Surgical Services Pre-Op and Recovery Room
• NRP:
  Family Birthing Center,
  Labor and Delivery
  , Neonatal Intensive Care,
  Mother/Baby Care
• PALS:
  Children Services, Cardiovascular Lab, Emergency Department, GI Lab, Interventional Radiology, Surgical Services Pre-Op and Recovery Room
