Responsibilities:
  • Responsible for walk-in and telephone triage.
  • Coordinates patient care through collaboration with the patient, family, and healthcare team.
  • Provides patient care by assessing, planning, implementing, evaluating, and documenting individual patient needs, actions taken, and patient responses.
  • Responsible for case management based on PCP panels, in collaboration with network providers, including nurse visits and telephone management.
  • Delegates or assigns aspects of patient care to healthcare team members according to their validated competency.
  • Completes documentation within scope of practice.
  • Performs other functions as required.
Qualifications:
  • Licensure:
    State of New Mexico or Compact State Nursing License
  • Degree from accredited academic nursing program (If a program is under conditional status, approval may be granted by system CNO)
  • Minimum ADN preparation with expectation of BSN completion within 5 years of hire
  • BLS required
